Swope Health Services provides a full array of services for ALL of our patients' health care needs.
At Swope Health Central, we are a "One-Stop Shop," meeting the following health care needs all under one roof:
Health Ambassadors
Program Goal:
Peer educators provide health care education, materials and Swope Health Services information to the community.
Eligibility:
Services provided to men, women and children.
Services Provided:
- Door-to-door and community events delivering:
- Health related educational information
- Swope Health Services information
- Free mammograms for eligible patients
- Assists patients in scheduling appointments
- Appointment reminder calls

Health Care for the Homeless
Program Goal:
To provide primary healthcare to homeless persons.
Eligibility:
- Verification that a client's housing situation fits one of four categories:
- Lives on the streets or in a shelter.
- Lives with a family member or friend due to an emergency situation.
- Is incarcerated or have just been released without any stable home plan.
- Has been homeless before entering a drug and alcohol treatment center and will be going to a shelter or transitional living program after completion.

Program Goal:
To decrease disease with the focus on health promotion and prevention of disease and disability through early detection.

Ryan White Program
Program Goal:
To provide primary health care and case management services to HIV+ patients.
Eligibility:
- HIV+
- Enrolled in the KC's Ryan White Program

W.I.C. (Women, Infants and Children) Program
Program Goal:
To provide nutritional counseling for low income women (pregnant, breast-feeding and postpartum), infants and children up to age 5.
Eligibility:
- Income guidelines (185% of the poverty level and below)
- Nutritional risk factors (determined by a Nutritionist)
Services Provided:
- Food vouchers
- Formula vouchers
- Nutrition counseling
- Breast-feeding education

Women & Minority Health Care
Program Goal:
To provide health insurance to indigent women, men, children and custodian fathers.
Eligibility:
- Women who are pregnant and unemployed or with wages under the federal guidelines.
- Custodial fathers who are unemployed or making wages under the federal guidelines.
- Families with children and with parents unemployed.
- Disabled individuals.
Services Provided:
- MC+/Medicaid Information and Applications.
- Permanent Medicaid (beyond the 45 days of temporary Medicaid).
- Follow up on denials.

Project Your Choice: SAMHSA
Program Goal:
- Assist clients to obtain and maintain housing using a "housing first" model
- Support clients in eliminating or reducing substance use
- Assist clients in accessing community and mainstream resources
- Assist clients to obtain employment or benefits
Eligibility:
- Chronically homeless individuals (as defined by HUD)
- Current or recent substance abuse
Services Provided:
- Intensive case management
- Referral to residential or outpatient drug and alcohol treatment programs
- Referral to counseling or other Behavioral Health services
- Referral for employment assistance
- Assistance obtaining housing
- Peer support
- Drug and alcohol free social activities
- Assistance obtaining benefits/entitlements
- Assistance accessing community and mainstream resources

Community Health Project
Program Goal:
To help provide access to care and education, prevent infant mortality, reduce low-birth weight babies and premature delivery among high risk women and teens in the urban core of Jackson County.
Eligibility:
Pregnant mothers residing in the Jackson County area that receive prenatal care from safety-net providers such as Truman Medical Center, Cabot Westside Clinic, Swope Health Services and Samuel U. Rodgers Health Center.
Services Provided:
- Case management
- Prenatal and parenting workshops
- One on one labor and delivery, education, coaching & facilitation classes
- Prenatal/Infant Nutritional counseling
- Breastfeeding counseling
- Baby supplies (clothes, diapers, formula, etc. after child’s birth)
- Transportation to and from prenatal exams, postpartum, well child checkups and follow up exams.
- Prenatal and parenting workshops
- One on one labor and delivery, education, coaching & facilitation classes
